Jeremy Moore

Jeremy Ryan Moore, 36 passed away on Thursday, November 22, 2018.

He was born on April 1, 1982 to Thomas and Kathleen (Holland) Moore in Waukegan, IL.  Jeremy graduated from Zion Benton High School.  He loved cars and the Chicago Bears, but his greatest love was fishing.

He is survived by his children, Arianna and Jordynn Moore, his parents, Thomas and Kathleen Moore, his brothers, Aaron (Samantha  Bouzeanes) Moore  and Joshua (Jocelyn Williamson) Moore, his sister, Amy (Derek Menier) Moore, a special cousin, Cory (Ally) Holland, his uncles, Michael (Joanne) Moore, his nieces and nephews, Janine Moore, Zachary Moore, Jace Menier, Connor Moore  and Tucker Moore  , his significant other Nicole Bouzeanes and her children, Jada and Ava Booth, as well as numerous aunts, uncles and cousins.

He was preceded in death by his Aunt Janine Holland.

Visitation will be held on Saturday, December 1, 2018 from 10:00-11:00 A.M. at Warren Funeral Home located at 1475 N. Cemetery Rd. Gurnee.  A Memorial Service will be Saturday at 11:00 A.M. at the funeral home.  Rev. Dan Stanford will officiate.
