Johnny Blake Phillips, 66 of North Chicago, passed away on Tuesday, May 21, 2019 at Highland Park Hospital.

He was born on July 23, 1952 to the late John Blakely and Eula May Lindsey.   On May 25, 1973 he married Mary Usher.  Johnny served in the U.S. Air Force and worked for National Gypsum Company.  Johnny loved spending time with his grandkids and bowling.  He was the number 1 fan of the Green Bay Packers.

Johnny is survived by his wife Mary, his sons; Johnny (Erica) Phillips, and Tony (Danisha) Phillips, his grandchildren; Jasmine Phillips, Quentin Phillips, Jada Phillips, and Jayla Phillips, his siblings; Chuck Phillips, Tony Casanova, Maurice Wiley, Mary Bailey, and Katherine Craigen.

He was preceded in death by his parents and his brothers; Robert and Demosthenese Phillips.

Visitation will be from 10:00 to 11:00 a.m. on Friday, May 24, 2019 at Warren Funeral Home located at 1475 N. Cemetery Rd. Gurnee, IL.  Funeral services will be at 11:00 a.m. on Friday at the funeral home.  Rev. Ezell Robins will officiate.

Burial will be at Warren Cemetery.
